Camshaft sprocket slipping on shaft?

Anyone ever experienced a camshaft sprocket slipping (ie rotating) on the shaft itself?

Causing of course a wrong valve timing etc.

On top of that, is there any way to check if that happened, are the punch marks on the sprocket where they should be in regard to the cam position?

After eating all its valves, my WRF 04 seems to have suffered such a fate too...
