Clutch Never Fully Engages?

Hey guys,

I have a 04' CRF250R that i got a few months back. Recently i started to notice that when i'm stopped and in first gear my bike slowly moves forward. It also makes a loud noise when switching gears from neutral into first. Also, once i get into first and try to stop its almost impossible to get it back into neutral with the engine running.

There are no other problems with the shifting just that i can tell the clutch doesn't engage properly. I have tried adjusting the clutch using the adjustments on the handlebars. When i adjust the clutch to grab any sooner it will be engaged when i have to let out. I can hear the bike cutting out because the clutch is engaged in the higher RPM's. I have it set now to the point where it doesn't affect when im actually riding, only takeoff. This obviously leads me to believe that its something with the clutch plates.

I opened up my transmission case and took a look inside. I had removed the metal plates and the friction plates but i really don't know what to look for as far as wear. There is no apparent wear on the housing that i could see. I also change my transmission oil at the recommended <15 hours so i know its not due to improper maintenance.

My question really comes down to what is actually happening here? Are my plates worn to where i need to replace them? Is there something else that maybe can be causing this? I really do not have much experience when it comes to bikes since this is the first one i have ever owned. Any help would be greatly appreciated since its getting annoying to have to shut my bike off in order to come to a stop.
