Cold Morning start problems...clunking...

i started my 650R up this-morning and it fired up and was clunking then it shut off. My vapor read 780 RPM's, so whats up with this? it only takes like 3 seconds of running to go away...my idle is around 1000-1200 on 45 degree mornings, then gets up to 1600 when the bike is warmed up....? any thoughts, ive just been holding the throttle open very slitely when i start it.

also what do you guys do about grips viberating? my hands hurt after 50+ miles.
