I was looking at installing hot cams on my 2002 426 to ease the compression, but after a little research into it I have found that there is a intake and exhaust cam, can any one tell me if I need to do both?

It depends what you're after.  Just to add the auto decompression feature, all you need is the exhaust cam.  Using the intake along with will gain you a small bit more power in addition.
